要办理《增值电信业务经营许可证〉申请浙江省增值电信业务经营许可证需提交如下材料:
1、公司法定代表人签署的《增值电信业务经营许可证申报表》(点击下载 增值电信业务经营许可证申报表)。
2、业务发展可行性研究报告和技术方案(根据申请的业务下载填写:申请信息服务业务请点击下载 信息服务业务可行性研究报告及技术方案.doc);申请其他业务请点击下载 业务可行性研究报告及技术方案 )。
3、若申请开展信息服务业务(包括互联网信息服务业务和移动网信息服务业务等)、因特网接入服务业务和因特网数据中心业务的,还需提交如下材料:
(1)网络与信息安全保障措施(点击下载 网络与信息安全保障措施)。包括:信息安全保密管理制度、用户信息安全管理制度、网络运行的安全保障制度、有害信息过滤的技术方案等安全保障制度。
(2)申请互联网信息服务业务的,需提交申请网站的域名证书及复印件(注:域名注册所有人应与申请单位一致)。
(3)申请互联网信息服务业务的,拟从事新闻、出版、教育、医疗保健、药品和医疗器械、文化(网络游戏)、影视等互联网信息服务的,在申请经营许可或者履行网站备案手续前,应当依法经有关主管部门审核同意后,提交有关主管部门审查同意的批文。
(4)从事互联网信息服务业务,拟开办电子公告服务的,应当在申请经营性互联网信息服务经营许可证或者办理非经营性互联网信息服务备案时,按照国家有关规定提出专项申请,填写《互联网信息服务业务电子公告服务专项申报表》(点击下载 互联网信息服务业务电子公告服务专项申请表 )。
(5)从事互联网信息服务业务,拟开办虚拟主机服务的,还需申请因特网接入服务业务(仅限虚拟主机服务),并提供互联网站接入的流程和制度(参照信息产业部33号令)。
4、为用户提供长期服务和质量保障的措施(点击下载 为用户提供长期服务保障的质量保障措施为用户提供长期服务保障和质量保障措施 )。具体可以根据信息产业部颁布的《电信服务规范》要求,制定本公司的具体措施。
5、公司的企业法人营业执照副本原件及复印件。
6、公司章程,公司股权结构及股东的有关情况。
(1)公司章程必须是在工商管理局备案的公司章程复印件;
(2)公司股东若是法人公司的,必须提供其工商营业执照和公司章程复印件;若是自然人的,必须提供其身份证复印件。
7、公司概况。包括:
(1)公司基本情况,拟从事增值电信业务的内部机构设置;
(2)公司技术力量和经营管理人员情况(要求提供公司工程技术人员(2人)、管理人员(2人)、财务人员(2人)的聘书或劳动合同复印件、学历证书或职称证书的复印件;
(3)与从事经营活动相适应的场地(要求提供房产证复印件或房屋租赁合同复印件及出租方房产证复印件等相关房产证明)、设施(开展业务的拟购设备清单)等情况。
8、公司最近经会计师事务所审计的企业法人年度财务会计报告或验资报告。根据规定,公司注册资金不得少于100万。
(1)所提供的审计报告或验资报告必须是一年以内的;
(2)审计报告或验资报告必须是国家批准营业的会计师事务所出具的,应提供会计师事务所的营业执照复印件;
9、证明公司信誉的有关材料。信誉文件是指证明申请公司具有的某种能力或客观状况的文件,如已获得电信业务经营许可证的复印件、获得政府授予某种荣誉的复印件、获得某种国家认证机构认证证明等。如没有,则不需提供。
10、公司法定代表人签署的公司依法经营电信业务的承诺书(点击下载 《依法经营电信业务承诺书》 );
11、国家规定的其他条件。

尚未获得企业法人营业执照的申请者,应当提交公司的企业名称预先核准通知书,无需提交第5、9项中规定的材料。第1项规定的书面申请和第10项规定的承诺书,拟成立有限责任公司的,应当由全体股东签署;拟成立股份有限公司的,应当由全体发起人签署。

注:上述所有材料,一律要求加盖公司公章,并用A4纸打印,按顺序装订成册。

Hair plugs,korean fashion wholesale, also known as hair grafts, implants, or transplants have been around for over fifty years to treat thinning hair. Initially, these surgeries did not work that well, but plastic surgeons have improved their technique in recent years.

Today surgeons use follicular unit transplantation. This method takes hair follicles from areas with thick hair that contain healthy hair growth, and grafts the tissue onto parts of the scalp with thinning hair. In past times transplants looked like cornfields, but with improved techniques, transplants now look very natural. In most cases the hair that is transplanted matches the surrounding hair perfectly.

Two types of hair plug surgery exist. In the first, the plastic surgeon removes a plug of tissue containing one to four follicles, which are then transplanted to the thinning area. The primary disadvantage of this method is it requires more time. Only 500 follicles can be done in a session, and the person may have to visit the surgeon several times to have a complete restoration.

The other method removes a swatch of scalp containing many follicles, which are then separated and transplanted individually. The second method requires stitches and leaves a visible scar. The individual must wear their hair longer to cover the mark.

Transplants for men have become fairly common and successful. However, transplants are not always a viable alternative for women. Male pattern baldness is an inheritable condition for men and women called androgenic alopecia, but behaves differently in men and women.

In men this condition manifests itself with isolated patches of baldness around the front hairline and crown. The hair on the sides and back of the head remains thick, and suitable for using in a transplant.

Unfortunately with women, the hair loss is not localized. Instead, the hair thins all over the scalp. This leaves little material to use for hair plugs for women. In some cases plugs can be used from other areas on the body, but this changes the individuals appearance. These plugs can be used for thickening, but they can't be used for bald areas.

In addition to androgenic alopecia, women may also loose their hair for other reasons. Anemia and thyroid problems can cause hair problems. If these conditions exist, they must be treated prior to any hair loss treatment. Menopause can also cause hair loss, and women may need to go on a hormone treatment before getting hair plugs.

Hair plug technology has made several advances, and the success rate has improved. Many men have had successful hair restoration with this method. Hair plugs for females are also successful, but used less often. The problem for women lies in finding good donor sites from which to get the transplant follicles.
